CLASS REGISTRATION

Please Note: Classes are filled on a first come first served basis. Class space is LIMITED, and
spaces will not be held without payment. NO cash refunds will be given. Refunds will be given in the
form of class credit or payment towards private lessons ONLY.

Instructions to sign up for Training Classes:

Step 1: Choose the class that your dog or puppy would like to attend.

Step 2: Check availability of that class by calling our office (973-625-2495) Monday -Friday or visit www.trainadog.com

Step 3: Complete and sign this Registration Form and email, snail mail, or fax it, along with your check or credit card information. Please include a copy of your dog's vaccination record or bring it to the first night of class.

Puppy Preschool is for puppies 12 weeks - 6 months old

What to bring to class every week:

1) SOFT TREATS - any soft treat that your dog enjoys. It is an important part
of your training class to find treats that your dog WANTS to work for.

2) COLLAR - an acceptable collar including: Nylon or Leather Buckle
Collars, Martingale Collars, Metal Choke Collars or Halti/Head Collars.
WE DO NOT ALLOW PRONG/PINCH COLLARS! (Please do not bring one)

3) LEASH - an acceptable leash including: 6 foot nylon or leather ONLY.
NO Flexi-Leads or Chain Leashes.

Please feel free to bring something to drink, but make sure it is in a plastic container, no glass.
Always feel free to ask questions during your class time. There are NO stupid questions!
